Form<\/strong><\/h2>\n\n\n\n<p>Use this form if you need to:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Renew your school\u2019s Numeracy \/ Literacy Program Resources and Assessment Agreement (QBSA and the digital Program Resources ); <strong>AND\/OR,<\/strong><\/li><li>Purchase FABS licences for Catch-Up Numeracy; <strong>AND\/OR,<\/strong><\/li><li>Purchase licences for the Numeracy Diagnostic Assessment.<\/li><\/ul>\n\n\n\n<p>You can check the expiry date of your school\u2019s current or previous application agreement or application licences through the following steps:<\/p>\n\n\n\n<ul
